CUSTOMER SERVICE FORMS

Beneficiary Change – The Beneficiary is the person who receives the money from an Annuity in the case of the death of the Annuitant or Contract Owner. Only the Contract Owner can change the Beneficiary.

Claim Forms – If you're the Beneficiary of an Annuity and the Annuitant or Contract Owner dies, you need to fill out a claim form. Claim forms vary from state to state. Click here for more information.

Direct Deposit – You can have periodic payments from your annuity deposited directly into a savings or checking account.

Lost Contract – Your contract is a legal document and it's important for you to have a copy. If you lose your contract, we will provide you with a replacement at no charge. Fill out the form, sign it and have an adult witness your signature. Mail or fax it to us and we'll send you a copy of your annuity contract.

Name Change – Owners or annuitants who've changed their name should fill out a Change of Name form.

Ownership Transfer – A Contract Owner may transfer ownership of the Annuity, however the transfer of ownership may be a taxable event, and an Internal Revenue Service Form 1099 will be issued reflecting any reportable amount. Please consult with a tax advisor before transferring ownership of an Annuity.

Release for Information – At times a Contract Owner may want to give permission for a third party to speak with Standard Life of Indiana about their Annuity. This form grants permission for the company to release information to designated parties.

Index Annuity Contract Change – Certain Index Annuities include the option of allocating a percentage of the funds to a fixed interest account. Each year, the Contract Owner may decide on the allocation of funds between the Equity Index Account and the Fixed Interest Account. This form is to request a change in this allocation.

Generic Withdrawal – Use this form to authorize a monthly distribution of earned interest.
